Why do you need a certificate of military service for citizens of Ukraine
A certificate in form 5 is issued to confirm the fact of military service or to confirm military service at the place of residence, military rank, date of conscription or conclusion of a contract.

Certificate on Form 5 is needed for:
- registration of benefits for military personnel and members of their families;
- receiving loans;
- obtaining accommodation in dormitories of military units.
Where can you get a certificate on Form 5 and how to do it
To obtain such a certificate, you need to contact the commander of your own unit with the corresponding requirement. The certificate itself is issued by the clerk of the combat unit.
To receive help on the form you need:
- submit a report addressed to the commander of the military unit;
- provide a passport or other identification document;
- in some cases other documents may be needed (for example, military ID, military ID).
